All routine office visits are by appointment only. To make an appointment, call the office’s main number at (860) 443-1891 between the hours of 9:00 am and 4:30 pm, Monday through Thursday and 9:00 am to 11:30 am, on Friday. Please give us at least 24 hours notice if you must cancel your appointment by calling the office’s main number.
Please remember to bring your insurance ID card(s) with you, as well as any medical information which will assist the doctor in your evaluation. These include MRI, CT scan or other test results and any type of medical records or reports from your doctor.
You will be responsible for any payment or insurance co-payment at the time of your appointment. Please contact our Billing Department if you have any questions at 860-443-1891 x 116.
Please feel free to bring a family member or friend with you to your appointment. Write down any questions you may have so we may better help you understand your condition, various tests or the bill for your visit.
If you need further assistance prior to your appointment, please feel free to call the office. We will be happy to assist you in any way we can.

Insurance Information
Our office participates with most major insurance companies in the area. If we participate with your insurance, we will bill your insurance company for you and wait for payment from your insurer. However, you will be responsible for any co-payment or deductible at the time of your appointment. If we do not participate with your insurance company, we request payment in full at the time of your appointment. We will then mail you a claim form, which you can forward to your insurance company for reimbursement. We accept personal checks, VISA, MasterCard, Discover and American Express.
Most insurance companies require precertification or prior authorization for radiology procedures and/or neurodiagnostic laboratory testing. Charges for these services can be discussed in advance with our office manager. We are happy to bill your insurance company for these services. However, it is your responsibility to be aware of your insurance company’s rules and regulations.
We try to stay informed regarding changes made by each insurer, and we will do our best to help you with any problems. It is ultimately your responsibility; however, to be aware of what facilities (i.e. hospitals, medical laboratories, physical therapists, etc.) participate with your insurance and inform us of any restrictions.
